Code review, cleanup and fix
The skew bit was leaking into Maxwell's immediate, so the lane offset is
now computed
the outpur reads, immediates of `0x80` or more, and map reads with an
immediate are rejected;
Dropped ISBEWR writes are now logged;
English comments and 3 unit tests
